Referral Process
A referral may come from the prospective patient, a family member, or a health professional. Senior Behavioral Health then provides a clinical evaluation – at no cost – to determine the level of treatment needed.

Inpatient Referrals
If coming from home, simply call 620-724-7399 and be ready to provide:
- Patient demographics (name, DOB, address & phone number, primary care physician & phone number)
- Presenting emotional/behavioral symptoms
- Psychiatric treatment history
- Medical diagnoses
- Insurance / social security ID numbers
- Medical Durable Power of Attorney / Guardian name & phone number (if exists)
- If coming from a hospital, nursing home, or assisted living home:
- Call 620-724-7399 and inform staff that you will be filling out the Patient Referral Form and faxing it to 620-724-5187, along with:
- Face sheet
- Medical DPOA/Guardianship
- Advanced Directive/Living Will
- Current MAR/TAR
- H&P
- Most recent labs
- Doctor’s order for psych eval/hospitalization
- Behavioral notes
- COVID-19 test results are done within 24 hours of admission to GMC/SBH. (unless the test is being done on our end).
