Referral Process
A referral may come from the prospective patient, a family member, or a health professional. Senior Behavioral Health then provides a clinical evaluation – at no cost – to determine the level of treatment needed.

Inpatient Referrals
- If coming from home, simply call 620-724-7399 and be ready to provide:
- Patient demographics (name, DOB, address & phone number, primary care physician & phone number)
- Presenting emotional / behavioral symptoms
- Psychiatric treatment history
- Medical diagnoses
- Insurance / social security ID numbers
- Medical Durable Power of Attorney / Guardian name & phone number (if exists)
- If coming from a hospital, nursing home, or assisted living home:
- Call 620-724-7399 and inform staff that you will be filling out the Patient Referral Form and faxing it to 620-724-5187, along with:
- Face sheet
- Medical DPOA/Guardianship
- Advanced Directive/Living Will
- Current MAR/TAR
- H&P
- Most recent labs
- Doctor’s order for psych eval/hospitalization
- Behavioral notes
- COVID-19 test results done within 24 hours of admission to GMC/SBH. (unless test being done on our end).
Outpatient Referrals
- Whether coming from home or a facility, call

They will gather the required information and schedule / perform an assessment determining whether the proposed patient fits criteria for entering the program. If found appropriate, the patient will then be scheduled for an intake session with the psychiatrist and, soon after, begin participation in cognitive behavioral group therapy.
