When to refer to our Cardiac Rehab Program:
- Recent Heart Attack w/in 12 months
- Coronary Stenting
- Stable angina or Stable CHF
- Coronary Artery Bypass Graft
- Valve surgery
- Heart transplant
What we need:
- Referral Form signed by a Physician (found above)
- Patient Demographics/Face sheet
- Qualifying Diagnosis
- A copy of PFT Results, Labs, medical history, latest progress notes, and medical list.
- Patient Insurance Information
